You can use the free Adobe Fill & Sign mobile app to add your signature to the form.
- If you do not already have the free Adobe Acrobat Reader application installed on your computer, you will first need to download it here before opening our documents.
- Download the application form using the link at the top of the form below
- Select ‘Fill & Sign’ in the menu on the right.
- Select ‘Fill & Sign’
- Select ‘Sign’ in the top menu to create your signature.
- Once the form is completed, you can use the document upload form below to submit it.
Once you have downloaded the form, you will need to print it, complete it and return it to us by either emailing it to [email protected] or dropping it into our office at 254 Charters Towers Road, Hermit Park. Please ensure you have all your supporting evidence for your Application which is listed on the first page of the Application. As you can appreciate, we will need the entire Application completed and all supporting evidence attached before we can commence processing your Application to ensure your Application is processed in a speedy timeframe.
